Another method to find out your bank balance is to visit your nearest ATM with your PNB debit card. Once there, you can follow these steps -

No, there is no restriction as to how many enquiries you can make. You just have to make sure that the gap between two subsequent enquiries from the same number is at least 45 minutes.
Charges upon SMS banking in PNB are levied quarterly. For Savings bank accounts, it is Rs.15/- per quarter, and for other accounts, it is Rs.25/- per quarter.
